Original and New Business Application

Applicant should read and understand the following:

Applicant must complete one of the following applications:

Allow 3 weeks for processing after submission of completed application package. There is a $500 application fee.

Change of Ownership Application

Applicant should read and understand the following:

Applicant must complete one of the following applications:

Allow 3 weeks for processing after submission of completed application package. There is a $500 processing fee.

Expansion of Premise Application

Applicant should read and understand the following:

Applicant to complete one type of application (corporation, limited liability, partnership or individual) Allow 3 weeks for processing after submission of completed application package. There is a $500 processing fee.

Temporary Liquor License Application

Applicant should read and understand the following:

Allow 3 weeks for processing after submission of completed application package. There is a $30 processing fee.

Keg Liquor License Application

Must be submitted at least five (5) working days in advance of proposed event. There is a $25 license fee.

Must be submitted at least ten (10) working days prior to event. There is a $10 daily fee or $250 annual fee.

Criminal Background and Credit Authorization Form

This form must be completed when any managers or officers change.
